Description:

Provides comprehensive treatment, education, and long-term management for infants, teens, and young adults with diabetes.

Offers a full range of primary and advanced diabetes education classes to patients and caregivers. Courses are led by certified diabetes educators and are available for both inpatients and outpatients.

Classes include:
- Basic self-management.
- Comprehensive education course (one week after diagnosis).
- Advanced self-management.
- Insulin pump therapy.
- Continuous glucose monitoring.
- Transition to adult care.

Hours:

Monday-Friday (Call for times as they vary)

Intake Process:

Call for intake/registration.

Program Fees:

All major insurance plans are accepted.

Financial assistance is available to qualifying families whose children have health care needs and are uninsured, underinsured, ineligible for a government program or otherwise unable to pay for their child!€™s medically necessary care based on their individual financial situation.

Eligibility:

Those with Type 1 diabetes, Type 2 diabetes, Atypical diabetes, Cystic fibrosis related diabetes, Diabetes insipidus, Steroid-related diabetes and
Transplant-related diabetes.

The diabetes education classes are available for both inpatients and outpatients.
